Peginterferon alfa-2bComplete Research Guide (2026)

Last updated 2026-06-30

TL;DR

Peginterferon alfa-2b (also known as PegIntron) is a peptide catalogued under Immune (PEG-IFN-α). Neutral reference entry; EU status: EU-approved prescription medicine.

What is Peginterferon alfa-2b?

Peginterferon alfa-2b is interferon alfa-2b conjugated to polyethylene glycol, which extends its half-life and allows less frequent dosing than conventional interferon. It is an approved biologic marketed as PegIntron (historically for chronic hepatitis C, usually with ribavirin) and as Sylatron for adjuvant treatment of resected high-risk melanoma.

Supporting evidence is high-quality and human, including the phase III EORTC 18991 melanoma trial and randomised registration trials in hepatitis C and B.

How does Peginterferon alfa-2b work?

Like unmodified interferon alfa-2b, the pegylated form signals through the type I interferon receptor and JAK-STAT pathway to drive antiviral, antiproliferative, and immunostimulatory gene programmes.

PEGylation slows clearance and sustains plasma exposure, giving more constant interferon receptor engagement between doses.

What does the research say about Peginterferon alfa-2b?

  • As adjuvant therapy it improved recurrence-free survival versus observation in resected stage III melanoma in the EORTC 18991 phase III trial. [1]
  • Combined with ribavirin it achieved higher sustained virologic response than standard interferon alfa-2b plus ribavirin in chronic hepatitis C. [3]
  • In HBeAg-positive chronic hepatitis B it increased HBeAg loss, with or without added lamivudine. [5]

Dosing context

This is not medical advice or a usage recommendation. Dosing figures are reported research context only, cited from the published literature.

Regimens have been weight-based and given by subcutaneous injection, typically weekly for hepatitis indications (with ribavirin for hepatitis C) and as a distinct multi-year weekly adjuvant schedule for melanoma under the Sylatron label.

This is contextual background only, not a dosing guide; dosing and monitoring must be set by a treating physician.

Side effects & safety profile

As an interferon-alpha product, peginterferon alfa-2b carries the same FDA boxed warning for fatal or life-threatening neuropsychiatric, autoimmune, ischaemic, and infectious disorders, and requires close monitoring with discontinuation for persistently severe events.

Common toxicities mirror conventional interferon: flu-like symptoms, fatigue, depression, neutropenia and thrombocytopenia, and thyroid or hepatic abnormalities.

It is prescription-only and must be given under specialist supervision with baseline and periodic laboratory and psychiatric assessment.

Stacking & combinations

Evidence-based combinations are limited to approved partners such as ribavirin (hepatitis C) or lamivudine (hepatitis B), and no combination with unapproved research compounds is supported.

Frequently asked questions

It is the same interferon protein attached to a polyethylene glycol chain, which lengthens its half-life so it can be dosed less often, usually once weekly.
